TRAVEL DOCUMENTS WAIVER

ACKNOWLEDGMENT OF NAME CONSISTENCY

I, the undersigned, acknowledge that I have instructed Curated Passage, LLC to book international air travel through their host agency, Largay Travel Inc., using a name that is different from my currently held passport. I confirm that I have applied for a new passport reflecting this new name.

RISKS AND RESPONSIBILITIES

I understand and agree to the following:

• Matching Documentation: Transportation Security Administration (TSA) and international border authorities require that the name on a flight reservation exactly matches the name on the valid passport used for travel.

• Issuance Delays: I assume all risk associated with the processing times of the issuing government agency. If my new passport is not received prior to departure, or if the name on the issued passport differs from the booking name, I may be denied boarding.

• Correction Fees: Most airlines do not allow name changes. Should a name correction be required, I am responsible for all associated costs, which may include airline change fees, fare differences, or the purchase of an entirely new ticket at current market rates.

• Ticketing Control: I acknowledge that Largay Travel Inc. serves as the ticketing agent and must adhere to strict airline tariff rules regarding name changes, which are often outside of their control.

RELEASE OF LIABILITY

I hereby release Curated Passage, LLC and Largay Travel Inc. from any financial or legal liability should I be unable to travel due to a discrepancy between my travel documents and my flight reservation. I agree to indemnify and hold both entities harmless for any losses, denied boarding incidents, or additional expenses incurred as a result of this booking request.
